Please download the fillable PDF referral from the options below. Please note:

  • As per recent updates from the Department of Health, referrer signatures are now NOT required
  • Be sure to include sufficient patient information, including patient contact phone number
  • Submit your patients referral using our Upload portal found on this page, or email your request to bookings@scr.com.au
  • Our bookings agents will contact your patient to make the appointment, this is why including their contact phone number is important
