Use your phone to scan or photograph your Referral Form before you start. You will be prompted to upload the image at Step 3.

Complete Form
You will be guided through the process by the Avatar. You may disable the voice at any time.

Confirmation
Upon submission of the completed form a notification will appear to confirm your requested appointment time, date and location.
SMS
24 hours before your appointment you will receive an SMS to reconfirm your requested appointment time, date and location.
